Simulate an encoder swap that goes wrong in three different ways (mixed-version index, prefix asymmetry, dimension truncation). For each, produce the diagnostic that identifies it.

Acceptance criteria

  • Three broken configurations, each with the measured recall drop
  • The diagnostic that isolates each, in the order a strong candidate runs them
  • A runbook entry: what you would check first at 2am, and why
