Who this page is for, and how to use it
For

Anyone about to write an artefact the manual asks for, a product manager drafting a pain register, an architect writing the spec, a QA lead building the bar sheet, a sponsor's two-number report.

Use it to

Find the template for the step you are on, copy it, fill in the angle brackets, and keep it as the artefact you hand to the next person.

How
  1. Pick your role in the left rail (or scroll: they are in journey order).
  2. Press Copy on the block. Paste it into your document.
  3. Unsure why a field is there? The line above each block links to the step that explains it.

What these templates are for, and how to use one

What they are for

  • Each is the document one step of the SkyWays PDLC produces and the next person needs: a register, a spec, a bar sheet, a report.
  • They carry the fields that get forgotten, so the next person never has to ask what you meant.
  • Kept in the repository, they are the evidence pack a gate is judged on.

How to use one

  1. Find your step.Pick your role, then the step you are on; the phase badge says where it sits, P0 to P3.
  2. Copy and fill.Press Copy, paste it into your document, and replace every angle bracket with your own material. Delete what does not apply; do not leave a placeholder.
  3. Hand it on.Check the "Good looks like" line, then give it to the person the step names. The step it comes from explains every field.

One of them, filled in

The pain register's first line at SkyWays, from six interviews and the Q2 ticket export:

# Pain register · SkyWays rebooking
Pain: Disrupted passengers wait an average of 38 minutes for a rebooking decision
Who said it: 6 of 6 transcripts (3 agents, 3 passengers)
Count: 240 a day; 11% codeshare
Cost per case: $9.40, measured
Source: Q2 ticket export, tickets tagged REBOOK
Owner of the number: Priya (PM)

One line per pain, every number with a source. That is what turns a vibe into something a sponsor can fund.

Templates or prompts?

Templates

Skeletons for the documents each step produces: a register, a spec, a bar sheet, a report. You fill the angle brackets and keep the file.

| Share of cases reviewed | <n>% | cycle 1 estimate |
| Review minutes per reviewed case | <n> | |
| **Net / day** | **$<n>** | |

Cycle-2 expectation: review share falls to <n>%, net becomes $<n>/day. Say this now,
so the cost story has a trajectory rather than a surprise.

## Autonomy, per action
| Action | Cost of ONE mistake | Reversible? | Level | Raise it when |

| <show options> | ~$0 | yes | acts alone | n/a |
| <same-day rebook> | $<n> | mostly | acts, monitored | <evidence> |
| <cross-partner rebook> | $<n> | with effort | acts, veto window | <evidence> |
| <issue refund> | $<n> | **no** | **named approver, every time** | does not rise without <x> |

## The door
Actions marked NOT reversible are one-way doors. They stay gated regardless of measured
accuracy, because the bar that would justify removing the gate is not reachable.

## Review cadence
Levels are reviewed <monthly> against evidence. An incident drops the level of the action
involved by one, automatically.

| <the reviewer> | independence | a checker: different model, or fresh adversarial context |
| <the planner> | the agent itself | the single agent |

## The escalation condition
We move to <orchestrator + workers> when, and only when:
- <a single context exceeds <n> tokens on <named case class>>, **or**
- <more than <n> sub-tasks must run in parallel AND a fan-out tool cannot express it>

Baseline measured <date>: <the number today>. Anything outside these two is a swarm
arriving by default.

## Process depth, per change

| <audited module> | yes | full | full | full | living spec + the full document trail |
| <regulatory rule change> | none | yes | yes | light | spec diff + validation |

**Rule:** the living spec is the backbone everywhere. The heavy document trail is
layered on only where the work is audited and multi-team. Depth flexes per change.

## Authority, before any token budget
| Tool | Band | What ONE wrong call damages | Where the control lives | The test |
|------|------|----------------------------|------------------------|----------|
| <search_flights> | R1 | nothing; read-only | permission scope: read only | <no write scope granted> |
| <draft_message> | R2 | a reply a person then sends | review before it merges | <a draft can never auto-send> |
| <cancel_hold> | R3 | a held seat | approve first | <no approval, raises> |

| <change_passenger_identity> | **R5** | identity | not delegated; no tool exists | <no such tool in the schema> |

**The ladder is about the action, not the diff.** A one-line change to <cap> is R4.

## Caps moved out of prompts
| Was, in the prompt | Now, in the signature | Tests written |
|--------------------|----------------------|---------------|
| <"never refund more than $400"> | <issue_refund(amount: Money, ...)>, raising above <CAP> | <test_over_cap_raises> |
| <"ask before rebooking"> | <rebook(..., confirmation: ConfirmToken)> | <test_no_confirmation_raises> |

The explanatory sentence **stays** in the prompt. It is policy and it makes the agent
behave well by default. The signature is what makes bad behaviour impossible once the
prompt has been talked past.

## The confirmation token
- Minted by: <the approver's screen>. Never by the model, never by a tool the model calls.
- Bound to: <booking id + amount + approver identity>
- Expires: <n> minutes
- Test: <a forged, reused or expired token raises>

## Ingest: everything the agent reads is untrusted
| Source | Tagged untrusted? | In the weekly injection suite? |

| <partner API free-text fields> | <yes> | <yes> |
| <uploaded documents> | <yes> | <yes> |
| <retrieved knowledge chunks> | <yes> | <yes> |

## Open decisions: hard or soft
Four questions, in order. **One "no" makes it hard.**
1. Can it be reversed cheaply once the build has started?
2. Can the build proceed behind a placeholder?
3. Is there a named owner and a date?
4. Does everything downstream survive if the answer changes?

| Decision | Q1 | Q2 | Q3 | Q4 | Verdict | Placeholder | Owner | By |

## 1 · Context layers, inherited downward
| Layer | Holds | Example files | Who edits |
|-------|-------|---------------|-----------|
| shared | org-wide standards, security, tone, glossary | <standards.md · security.md · tone.md> | <platform> |
| domain | the business model for this area | <booking-model.md · fare-rules.md> | <you> |
| product | this application's architecture and APIs | <architecture.md · apis.md> | <the product team> |
| task | this feature's spec, bar and file list | <spec.md · bar.md · files.md> | <the bolt's owner> |

**Rules**
- A new product writes ONLY product and task. It inherits the rest.
- A shared change is one commit, and every product picks it up.
- An override declares its reason and its scope. A silent override is a drift.
- Send the slice a task needs, never the whole stack: <40k> tokens becomes <2k>.
- The domain layer is the one teams forget, and it holds most of the reuse.

## 2 · The server for <system>
| Primitive | Controlled by | Entries |
|-----------|---------------|---------|
| resources, read-only data | the application | <booking_record · passenger_history · fare_rules> |
| tools, open reads | the model | <search_flights · check_availability> |
| tools, gated writes | the model, within the contract | <rebook (CONFIRM)> · <issue_refund (CONFIRM, amount at or below the cap)> |
| prompts, templates | the person | <draft_disruption_reply> |

- Transport: <Streamable HTTP in production, stdio for local development>
- Auth: <as the specification's authorization framework requires>
- Trace: every tool call logged (input, decision, output, model version)
- **Rejected:** <one broad manage_booking(action) tool: any path through it could cancel>

M applications times N systems becomes M + N. Build the server once and any compliant
client plugs into it.

## 3 · The chain, and where the checkers go
> <0.9> x <0.9> x <0.9> x <0.9> = **<0.66>**: multiply, never average

| # | Step | Kind | Checker? | Why |
|---|------|------|----------|-----|
| <1> | <read the booking> | best-guess | no | <low stakes, caught downstream> |
| <2> | <choose the flights> | best-guess | **yes** | <costly to get wrong, easy to miss> |
| <3> | <compute the fare> | exact | no | <a unit test, not a checker> |
| <4> | <draft the message> | best-guess | **yes** | <false claims and tone, invisible to the drafter> |
| <5> | <rebook> | consequential | no | <a confirmation, not a checker> |

**Independence**: each checker runs on <a different model> OR <the same model in a
fresh context with an adversarial brief>. Never a self-review, and never inside the
context that produced the output.

**Checker brief, step <n>:** <Find what is wrong with this. Do these options satisfy
every stated constraint? List the violations. Do not rewrite.>

**Re-draft cap:** <2> rounds, then escalate to a person.

## Caching layout
[tools] [system] [shared context] [domain context <- cache marker] [task] [the request]
  stable ---------------------------------------------------------> | volatile

- The cache matches an EXACT prefix, in the order tools -> system -> messages, up to
the marked block. The changing request goes last, always.
- Documented multipliers, read September 2026: five-minute write 1.25x input ·
  one-hour write 2x · read 0.1x · <Fable and Mythos 5.1 read at 0.025x>
- cost with cache = ( w + 0.1 x (N - 1) ) x T x p, against N x T x p without.
**Break-even is the second use.** At N = 1 it costs more than not caching.
- Window: <five minutes> because <calls arrive seconds apart and each hit refreshes it
free>. Choose one hour when <a call every twelve minutes means the short cache has
always expired and every call pays a write>.
- Minimum cacheable block: <about 1,024 tokens, model dependent>

| What breaks the cache | Present here? |
|-----------------------|---------------|
| The request placed first, for emphasis | <no> |
| A timestamp, request id or session id inside the cached block | <no> |
| A model switch mid-task (the cache is model-scoped) | <forbidden in the team rules> |
| Fewer than the minimum cacheable tokens | <no> |

## Routing and the breaker
| Complexity score | Tier | Example |
|------------------|------|---------|
| below <0.3> | <cheap> | <is my flight delayed?> |
| <0.3> to <0.7> | <mid> | <am I owed a hotel under the regulation?> |
| above <0.7> | <frontier> | <missed connection, lost bag, wedding tomorrow> |

- MAX_LOOPS = <5> on every loop, then a hard stop and a hand-off to a person.
- A per-transaction token cap, next to the breaker.
- Alert on cost per case at **3x** the ratified figure, from the daily per-call log.

## The trace row
ts · input (passport ****1234, card ****9902) · tools called · decision ·
model version · approver · cost · cache tokens read

- **Redact, do not omit.** Masking keeps the decision replayable and the identifier out
of the store. Omitting the field breaks the audit instead.
- Retention: <90 days hot, then aggregate>. Traces are production data; guard them like
  production.
- Mark cache hits and EXCLUDE them from the latency alert, or a fast response is
reported as a suspected failure and someone proposes switching the cache off.
- Test: <a passport number never reaches a row>.

## Context layers. READ THESE BY PATH. Do not paste them into a prompt.

- /context/domain/<booking-model>.md the domain vocabulary
- /context/product/architecture.md this product, plus ADR-001..ADR-<n>
If a rule here disagrees with a layer, the layer wins and this file is out of date.

## Conventions
- Exact work is a function with a unit test. Never compute money in a prompt.
- Every tool that writes takes a confirmation token. Reads do not.
- One model per task: the cache is model-scoped and a switch discards it.
- A new dependency is an issue first, never an addition made in passing.

## Commands: every one of these has been run. Use them; do not improvise.
| To do this | Run |
|------------|-----|
| Unit tests | `<make test>` |
| The golden slice for one bolt | `<make golden SLICE=codeshare>` |
| Lint and types | `<make check>` |
| Local stack up | `<make up>` |

## Never touch
- `<config/caps.yaml>`: the authority budget. Changing a cap is an R4 change.
- `<src/tools/refund/**>`: two named reviewers, every time.
- `<migrations/>`: never edit an applied migration. Add a new one.
- `<src/trace/redact.py>`: security reviews every change here.

## Ask before
- Adding a tool to the agent's tool list
- Editing anything under `<src/prompts/>`: it re-runs the injection suite
- Touching a golden-set file. Those belong to QA, not to this repo's authors.

QA lead: 8 templates, P0 to P3

1. Proof map and bar sheet P1 Define

  • Use it whenP1, the day the architect's step map exists and before a single test is written.
  • You produceProof map + bar sheet, owned by QA lead, with the product manager on saving and damage.
  • Start withTag every step on the architect's map exact, best-guess or consequential.
  • Good looks likeEvery step on the architect's map carries a tag, the proof that tag owes and the way it fails; every best-guess slice carries a bar with the two money figures it was derived from. One page, no step untagged.
  • Explained instep 1, Define
Proof map and bar sheetQA step 1 · Proof map + bar sheet
# Proof map and bar sheet · <feature>
_Owner: <name> · Source: the architect's step map <version> · <date>_

## What each step owes
| # | Step | Kind | The proof it owes | Fails how |
|---|------|------|-------------------|-----------|
| 1 | <fare difference> | exact | a unit test, green or red | loudly, in CI |
| 2 | <choose the alternative> | best-guess | measured share on <slice>, vs its bar | **fluently**: confident and wrong |
| 3 | <issue the refund> | consequential | over-cap raises AND no-confirm raises | silently, until money moves |
| 4 | | | | |

A step you want to give two tags is two steps. Send it back to the map.

## The bar, per slice
**N = damage ÷ saving** · **bar = N ÷ (N + 1)**

| Slice | Saving per right case | Damage per wrong case | N | Bar | Held? |
|-------|----------------------|----------------------|---|-----|-------|
| <same-day> | $4 | $4 | 1 | 50% | no |
| <codeshare> | $9 | $36 | 4 | 80% | no |
| <refund, unheld> | $12 | $600 | 50 | **98%** | no |
| <refund, named approver> | $12 | $30 | 2.5 | **71%** | yes |

**The hold is the lever.** It lowers the damage, so it lowers the bar. Shipping at 71%
with a person confirming the charge beats waiting for a 98% you will never reach.

## Consequential steps: the two tests
| Tool | Cap, in the signature | Who mints the token | over-cap raises | no-confirm raises |
|------|----------------------|---------------------|-----------------|-------------------|
| <issue_refund> | $<n> | <the approver's screen> | | |
| <issue_credit> | $<n> | | | |

A boundary that is not one of these two tests is a sentence in a prompt, and a model can
be talked past a sentence.

## Where the numbers came from
- Saving per right case: <source, date>
- Damage per wrong case: <source: the leak as well as the transaction>
- Agreed with <PM name> on <date>

## Open
| Missing | Who owns it | By when |

Shadow comparison and expansion-gate evidenceQA step 7 · Shadow comparison + expansion-gate evidence
# Shadow run · <feature>
_Window: <start> to <end> (<n> days, fixed on <date> before the run) · Owner: <name>_

## Agreement, per slice
| Slice | Decisions compared | Agreed | Agreement | Threshold | Met? |
|-------|--------------------|--------|-----------|-----------|------|
| <same-day> | <n> | <n> | <n>% | 95% | |
| <codeshare> | <n> | <n> | <n>% | 95% | |
| <refund> | <n> | <n> | <n>% | n/a, excluded, stays gated | none |

**Money actions are excluded from automatic agreement and remain gated regardless of
what this table says.** Every other threshold here is tunable; that one is not.

Any slice under <n> compared decisions is **unproven**, not failed. Say which.

## Shadow never writes
| Assertion | Where it runs | Last green |
|-----------|---------------|-----------|
| no write call from the shadow path | <the nightly job> | <date> |
| shadow decisions land in <the log>, not <the ledger> | <the nightly job> | <date> |

## Disagreements: themes, not cases
| Theme | Count | Agent right / desk right / ambiguous | What it changes |
|-------|-------|--------------------------------------|-----------------|
| <theme> | <n> | <which> | <golden cases / rubric line / spec defect / nothing> |

Read in full by <name> for the first <n> days. Ambiguous themes are rubric work.

## Days of evidence, per share
days = cases needed ÷ (traffic share × cases per day) · cases per day = <n>

| Slice | Cases needed | At 5% | At 25% | At 100% |
|-------|--------------|-------|--------|---------|
| <codeshare> | <n> | <n> days | <n> days | <n> days |

Any slice needing more than <30> days at 5% needs a bigger starting share or a hold,
and that is a decision to take now rather than in week five.

## Widening: conditions, never dates
| Step | Condition |
|------|-----------|
| to 5% | threshold met on <slices>; rollback rehearsed on <date> by <name> |
| to 25% | live lower bound ≥ bar on <slice> for <n> consecutive days |
| to 100% | as above, plus zero <class> incidents and the injection suite green |

## Verdict
<open the expansion gate for <slices> | extend the window by <n> days | do not proceed, because ...>

| <% refund vs credit> | <n>% | <n>% | <n>% | <n>pp | <n>pp |
| <% propose vs escalate> | <n>% | <n>% | <n>% | <n>pp | <n>pp |

**Two thresholds, because one is not enough.**
- Step: alert at **5pp** week over week. Catches a jump.
- Level: alert at **over 6pp** against the frozen baseline. Catches the slow slide that a
week-over-week rule never fires on: under 2pp a week moves 13pp in seven weeks.

A drift alert **re-opens the release gate automatically**. Last triggered: <date / never>.
Baseline re-frozen only by <name>, and never to make an alert go away.

---
# Incident · <id> · <date>

**1 · The question**
Which **enforced** control would have made this impossible?
(Not: who wrote the prompt. That question costs an hour and changes nothing.)

**2 · Layers claimed, honestly classified**
| Layer | Claimed | Reality: enforced / a request / absent | Would it have stopped it? |
|-------|---------|----------------------------------------|---------------------------|
| <input marked as data> | yes | <absent> | no |
| <the prompt's policy> | yes | **a request** | no |
| <a $400 cap> | yes | <a request: in the prompt only> | **yes** |
| <a named approver> | yes | <a request: in the prompt only> | **yes** |
| <an alert on the trace> | yes | <absent> | no, it reports afterwards |

Two layers that fail together are one layer. A rule that lives only in a prompt is a
request, and a model can be talked past a request.

**3 · The fix**
| Proposal | Closes the path, or lowers the probability? |
|----------|--------------------------------------------|
| <reword the prompt> | lowers the probability: the next attempt is worded differently |
| <add an alert> | neither: that is detection, not prevention |
| **<cap and confirmation token in the tool signature>** | **closes the path** |

**4 · Autonomy**
| Action | Level now | Level after | Condition to restore |
|--------|-----------|-------------|----------------------|
| <issue_refund> | <2> | <1> | <a 14-day shadow run at or above threshold> |

**5 · Feed forward**: four artefacts leave the room
- **Golden cases:** <6> new, in <slice>, each with its expected refusal and reason code
- **Injection payloads:** the string, tried from every entry point
- **Amended decision record:** <ADR-nnn> now defines *enforced* as **in the tool signature**
- **P0 brief:** pain · evidence · the missing enforced control · fix · value
